With many older adults living with one or more chronic conditions, senior living communities play a critical role in supporting residents’ health, independence, and quality of life. Effective chronic disease management requires a coordinated, whole-person approach that integrates medical oversight, functional support, and psychosocial care.

Independent Living and Assisted Living communities are uniquely positioned to bring together healthcare partners, staff, residents, and families to create proactive, collaborative care models. This session presents practical strategies for managing chronic disease, reducing hospitalizations and unnecessary transitions in care, and addressing the physical, emotional, and cognitive challenges that impact resident well-being.

Participants will gain a structured framework for implementing integrated care plans, strengthening community and health system partnerships, and delivering supportive programming that helps residents thrive in place while maintaining dignity, vitality, and independence.
