Value-Based Care: Getting Senior Living a Seat at the Table
As CMS advances its commitment to move all Medicare—and most Medicaid—beneficiaries into accountable care relationships, including Medicare Advantage plans and a range of other value‑based care arrangements, aging services providers are feeling the pressure to transform. Despite delivering care innovations that improve outcomes and reduce costs, these providers are too often left without a meaningful share of the value they help create.
Attendees will learn about efforts to ensure aging services providers have a true seat at the table in value‑based care discussions—across Medicare Advantage, Accountable Care Organizations, and the CMS Innovation Center. We will also discuss opportunities emerging within these models, explore what meaningful participation should look like, and revisit earlier policy ideas to assess whether they still hold promise in today’s rapidly evolving landscape.
Join us for a forward‑looking dialogue about the pathways, partnerships, and policy changes needed to ensure aging services providers can both contribute to—and benefit from—the future of value-based care.

Vice President of Integrated Services & Managed Care at LeadingAge
Nicole Fallon is the Vice President of Integrated Services & Managed Care at LeadingAge. In this role, she provides strategic leadership and member support to LeadingAge’s aging service providers on health policy with a focus on Medicare and Medicaid managed care programs, alternative payment and risk-based models, and quality and value-based care. She brings over 30 years of extensive and diverse health policy experience to her role, previously serving as a consultant at CliftonLarsonAllen and UnitedHealth Group.
