Health Dimensions Group® Managed Community Caregiver Wins LeadingAge Minnesota Award
MINNEAPOLIS, Minn. (Feb. 11, 2026) – Health Dimensions Group (HDG), is proud to announce that Kali Schlosser, social services designee at Frazee Care Center in Frazee, Minn. has been awarded the 2026 Stars Among Us District C Caregiver of the Year Award by LeadingAge Minnesota.
Each year, LeadingAge Minnesota’s District Caregiver of the Year Award recognizes individuals in older adult services who demonstrate exceptional commitment to seniors and enhance the quality of life of those in their care. Frazee Care Center, Frazee’s premier senior living community, is managed by HDG, a leading senior living, post-acute, and long-term care management and consulting firm based in Minneapolis, Minn.

Kali began her career at Frazee Care Center in 2021 as a certified nursing assistant (CNA) in training. She quickly earned her CNA license, and her passion for resident care led to her transition into a trained medication aide (TMA). After four years, she began working as a social services designee. Whether it’s administering hands-on nursing care as a licensed TMA/CNA, serving as social services designee, stepping in to run resident activities, helping with dietary services, stocking supplies, or performing housekeeping duties – Kali is never above any task. She makes every resident feel at home.
