Position Overview: We are seeking a compassionate and experienced RNAC (Resident Nurse Assessment Coordinator) to join our team at Simpson Senior Living. The RNAC plays a crucial role in assessing the health needs of our residents and coordinating individualized care plans to ensure the highest quality of care.

Key Responsibilities:

  • Conduct comprehensive assessments of residents' health status, including physical, mental, and emotional well-being.
  • Collaborate with interdisciplinary teams, including physicians, nurses, therapists, and social workers, to develop personalized care plans for residents.
  • Ensure accuracy and compliance with regulatory requirements in resident assessments and care planning documentation.
  • Coordinate and facilitate care planning meetings to discuss residents' progress, goals, and care plan adjustments.
  • Provide ongoing education and support to staff members regarding assessment processes, documentation requirements, and best practices in resident care.

Qualifications

  • Valid Registered Nurse (RN) license in Pennsylvania.
  • Minimum of 3-5 years of nursing experience in a senior living or long-term care setting.
  • Certification in Resident Assessment Coordinator (RAC-CT) preferred.
  • Strong knowledge of Medicare/Medicaid regulations and reimbursement systems.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
  • Detail-oriented and organized, with the ability to prioritize tasks and manage multiple responsibilities effectively.
